Patient has a habit of picking at own face, causing open area that become infected.
Patient has a open area 2cm by 1cm with a depth of 2mm.
I wanted to chart it as a facial ulcer and stage it but was told that ulcers only occur on pressure areas and cannot be staged on the face.
You are correct, the NPUAP pressure ulcer staging classification is intended for use with Pressure Ulcers only, however, the wound you have described can still be called an ulceration just with no stage. In addition, when describing non-pressure related ulceration’s, terminology should include whether the wound is partial or full thickness.
